A Clinical Evaluation of Ectopic Pregnancy.

Abstract
|
|
|
This study is a clinico-statistical analysis of 174 ectopic pregnancies who were admitted and treated at the Jeonbug National University Hospital from January 1, 1978 to December 31, 1982.
The results are as follows ;
1. The incidence of ectopic pregnancy was 7.0%. There has been no appreciable difference in the incidence of ectopic pregnancy.
2. The ectopic pregnancy was most frequent in 1!5^39 years of age, comprising of 78.0%.
3. Nulligravida wN 9.2%, nullipara 21.3%, and the patients who bad experienced more than once of the artificial abortion and spontaneous abortion were noted in 60.3% - and 19.0% respectively.
4. A history of previous laparotomy was 21.8% and pelvic inflammatory disease 24.1 %.
5. As to symptomatology, lower abdominal pain was encountered in 98.3%, vaginal spotting in 67.8%. On pelvic examination, abdominal tenderness was noted in 81.0%, adnexal mass to 40.2%.
6. The clinical manifestaions were appeared in 41.4% from L.M.P. to next 4~6 weeks.
7. On previous treatment, operation was recommended in 33,9%, curettage was performed in 10.9%, medical treatment in 8.6%, operation wen-, recommended after curettage and medical treatment in 6.9% and 5.2% respectively.
8. On initial systolic blood pressure and pulse rate, the normal range were in 58.6% and in 69.0 % respectivery.
9. Hemoglobin level less than 10 mg/dl was found in 50.6% and the amount of free intra peritoneal blood over 1,000 cc in 47.6% and more than 3 pints transfusion was needed in 52.9%. WBC count over 15, 000/mm3 was in 27.6%.
10. Gravindex test was positive in 49,5%, while culdocentesis being positive in 94.6%.
11. On implantation site, tubal pregnancy was detected in 99.4% (ampulla 71.7%, isthmic 20.2%, fimbriae 6,4%) and cervix in 0.6%. The tubal pregnancy was terminated by rupture in 59.0%, by abortion in 41.0%.
12. 86.2% of total cases was treated by salpingectomy, 8.0% by salping000phoreoctomy, Tice incidental procedure were tubal ligation in 42..95%, adhesiolysis in 8,0%. 13. Of 174 total
cases, there was no death.
